16 - PRESCRIPTION POLICY

Per FDA regulations, some of the items that we sell require that we have a prescription on file before dispensing. Those items include CPAP and Bi-Level Machines and Interfaces (Nasal Pillows, Nasal Masks, or Full Face Masks) and Portable Oxygen Concentrators (POC's). We will advise you at checkout if a prescription is required for any of the items in your order. You may download and use any of the PDF prescription forms that we provide for each item category, or you may provide a prescription from another source. The prescription must contain the required elements for use of a particular type of machine. We will review the prescription for completeness prior to shipping your order.

17 - Payment & Shipping

M4 CPAP and More Ships orders within the United States and its territories (including Puerto Rico, the U.S. Virgin Islands, Guam, etc. and military boxes). We do not ship to international destinations. Our regular business hours are Monday - Friday, 8:00 a.m. to 5:00 p.m. Pacific Time. We will generally ship out the same day if your order is placed by 3:00 p.m. Pacific Time. If for any reason there is a delay in the shipment of your order, we will contact you using the information provided by you at the time of check out.
We ship orders via the United States Postal Service and United Parcel Service. Orders of $99.00 or more ship FREE when using economical ground transportation. When expedited shipping is requested, there is a shipping charge for items over $99.00. You may choose from a variety of shipping options. Most orders under $99.00 will be shipped using the USPS First Class or Priority Flat Rate option. The least costly method of shipping will be calculated for you at checkout, before your order is confirmed. You may then choose an expedited shipping option which will increase your shipping charge accordingly.

18 - Return Policy

M4 CPAP and More accepts returns of any unopened product within 10 days of the original purchase date for a full refund or exchange. Unless the return or exchange is due to an error on our part, we cannot refund shipping charges.
Prior to return shipping, please obtain a return authorization (RA) number from us. You may either contact us by phone at (800) 696-2968, or email us at cs@M4cpap.com.
Due to the personal nature of some items that we sell, and current FDA regulations, we cannot accept return of certain items once the product packaging has been opened. CPAP Machines, Bi-Level Machines, Home Ventilation Machines, Humidifiers, and Interfaces (Nasal Pillows, Nasal Masks, or Full Face Masks) cannot be returned to us once the original product packaging has been opened, regardless of whether the machine has hours used or not.
Most of the products that we sell are covered under manufacturer warranty. If you wish to return an item under warranty, due to manufacturer defect, please contact us as directed above, to determine if the product is still covered under warranty. For defective machines, we will contact the manufacturer for you and arrange for a replacement. For all other warranty products, we will provide you with an (RA) number for return shipping. Once we have received your returned product, and it is determined to be defective, we will exchange the defective product with a new one. If we determine that the product is not defective, we will return the product to the customer and charge the credit card for return shipping.
No credits or refunds will be issued until we have received the returned product at our facility and it has been inspected by our staff. All returned products, except those being returned for manufacturer defect, must be in original condition and packaging; otherwise additional charges may be applied.
